    To Whom It May Concern,I am writing to file a formal complaint against the Anacostia Health Center located at **************** regarding an issue I encountered on September 13, 2024. On that day, I visited the center due to tooth pain stemming from an accident at another company. I told the dentist, ****** L. *****, DDS, that I bit a s**** that was in food prepared by a hotel, which resulted in tooth pain and damage. After my appointment, I received a visit summary, which contained incorrect information, including a statement that I did not make.I have made multiple attempts to resolve this issue. I called the health center several times but received no answer. I also visited the center on several occasions to request a corrected visit summary and an itemized bill. Unfortunately, the dentist has repeatedly refused to make corrections to the paperwork. Additionally, the manager assured me that they would follow up with me on the same day, but I never received any communication.Throughout this process, I have experienced extreme rudeness from the staff, and my concerns have been completely disregarded. This situation is now delaying my insurance claim and preventing me from receiving the necessary treatment for my condition.I would like this matter to be handled as soon as possible so that I can receive the corrected visit summary and an accurate bill for the services rendered on September 13, 2024.Thank you for your attention to this matter.

    1. I (TG) called unity health care (uhc) in Feb 2024 and spoke with a trained hired representative/worker of a company (rep) that informed me (TG) that all uhc locations offer walk in services. (uhc has call logs on record)2. March 11, 2024 10:30am I arrived at *** ******************* location to be seen, served, accommodated and assisted with uhc accredited services. #2 ******, next to her other two coworkers #3 & #4, called me over and asked if I had an appointment. I informed her I didnt, she said *** was fully booked and a doctor called out. I said OK but doesnt uhc take walk ins. ****** (uhc rep) said uhc ******************* isnt taking walk ins today 3. I told ****** that I called the mainline whom said that all uhc locations take walkins. ****** repeats herself then I ask if any other locations were taking walkins today. ****** replies she doesnt know and she tried calling other locations but couldnt get through to them all morning. I now state that this information sounds ridiculous. So I call the hotline immediately while still sitting across from ****** 4. Im literally appalled at the fact that I was lied to to my face, humiliated, violated, refused services, not accommodated or assisted, nor protected by the other two coworkers sitting right next to her assisting no one. WATCHING ME GET TURNED AWAY. This type of ongoing abuse is triggering to any individual whom is anti bullying and disrespect. 5. ****** didnt ask for my ID or even my insurance information, you want to know why? Because uhc does not take walkins. 6. I go outside to speak with phone rep whom informs me uhc rep lied to me and to turn around and go back inside because all uhc locations take walkins up until 4:20p 7. I return to uhc and was assisted by #5 whom called ****** (uhc director)8. Discussion with ****** while on-site 25yr old officer with glasses was present 9. *********************** phone calls and emails 10. Emails with ceo *********************** and operations person ***********************, phone calls 03/14/2024
