One of my children attended 4D Vision Gym until the end of June 2022. Two of my children attended 4D Vision Gym until January 2022. ****** **** ***** covered the expenses of Vision Therapy and we paid 100 a month per child out of pocket. I had difficulty getting into my HSA account online but when I did so a few months ago I discovered 4D Vision Gym kept taking out or trying to take out money from our HSA account depending on available funds for over half a year following our discontinuation of services with them. These are the dates and amounts 4D Vision Gym has taken out of my family HSA account since my children stopped going there. JULY 2 2022 $100 AUG 2 2022 $100 Jan 3, 2023 $100 Feb 3, 2023 $236.39 Feb 3, 2023 $150 ****** told me we needed to speak with 4D Vision Gym directly. They were not available by phone and claimed they called when they had not. They finally told my husband via telephone that the money 4D vision gym took out of our HSA account would not be refunded until ****** paid for uncovered bills. My husband was also told they would email him a list of services that were not paid. They did not. I again called Anthem. ****** was unable to reach them by telephone. ****** said that my children ********, ********* and ******** visits were "processed and paid with 0 dollar responsibility to the member". Please help me recover the money 4D Vision Gym has taken out of my HSA account so that I can pay current medical bills. Thank you, **** Lomasky
