    I saw ********** on 12/26 for three crowns. I have three temp crowns in my mouth currently and they have been there since 12/26/23. I had an appointment in January (i got COVID and had to reschedule) and then I was in a car accident (car is still in the shop) in January and had to reschedule again. With that, the practice manager, ******, stated via text (I have copies) I had to pay $250 out of pocket to make an appointment because I had cancelled the last two. I cancelled and went to another dentist to have the temp crowns completed; however, it appears that ********** billed for services NOT yet rendered and refuses to give back the money to Delta Dental so I can have my teeth completed by Serenity Dental. In addition to the insurance fraudulent billing, ********** also took $2400 from me out of pocket and now my bank is investigating him as well and they have taken the money back from ********** during the investigation. I just need my teeth fixed. The temporary crowns are causing major damage while we wait. A complaint has been filed with ********************* Provider as well. They will investigate improper billing practices by the dental office. It is also important to note that I was contacted by the office AFTER they refused to see me without the $250 payment that they do not have all the new patient information forms filled out and are asking that I NOW complete them. Someone needs to manage this office or just shut it down. Poorly managed practice!According to Delta Dental, this dentist (***) pre-billed for three crowns. The first problem is that he made ONE large huge crown for three teeth. I cannot floss between my teeth as they are hooked together like one tooth; however, he pre-billed like the task was completed with three permanent crowns. That did not happen. NOW, I have to file a complaint with the insurance company.

    In regards to the case of ****** "******" ******, this patient originally had work done on the lower left which consisted of crowns and buildups on 18, 19, 20 and a composite on #**. The Initial preparation date was 12/26/23. The patient canceled multiple appointments. My office made multiple attempts to get the patient rescheduled, however, the patient declined.  Once I was informed the patient would not be returning for crown seats, the refunds were sent to the patient and the insurance company, Delta Dental of *************. If supporting documentation is needed I can provide. I have not attached any documents as they contain patient personal information.  As of now, I have done all I can for this patient and the requests.
