My family of five have a loyal customer for over 10 years. Past few years, **************** performed my eye exams for the contact lenses and my copay has been consistently $40 which has been covered by the **** Insurance. This year for some reason unexplained before the exam, he has billed me using a different billing code "Premium Evaluation" for $200 which is not covered by my insurance. I was not explained of this procedure nor was I informed that this would be out of pocket expense. I did not consent to this "Premium Evaluation". I would not gone through this exam had I known that's not cover by my insurance. There was no special exam done this year than the previous years. This year my lense prescription remained the same but **************** billed me for more money using different billing code. I call the **** customer service and they advised me to reach out to the provider to get the money back on the procedure. Unfortunately I already payed the balance. When confronted with ****************, his explanation was that they incorrectly billed for the past 3 years and that I got a discount. Another explanation he shared with me is that if changes the code, he would be committing a fraud. This is a ridiculous explanation. He was firm and that he would not be providing a refund. What ki So is this mean, he committed fraud for the past 3 years??? What a ridiculous excuse! I did not consent to $200 "Premium Evaluation". I need to be refund that I overpaid. He was not cooperative and nor was he negotiable.
