      Deliberately misled customer that social was only needed on fingerprint cards. Submitted form for electronic fingerprints without social security number and gave fingerprint cards back to customer. Staff member called business owner who inferring that she would follow directions of *** asked customer to return after hours to be told that this would not be rectified unless process was completed again and payment would occur again. . I could have been told this the first tome but instead was asked to come back.

      ******** D ******* came to the office on 05/02/2025 to request a live scan service. She didn't fill out the social security number portion, and therefore I did not submit one. 

      What I submit is solely based on what the applicant fills out; therefore, it is her responsibility that it got denied. If needed, she can get a free resubmission done once she receives the letter.

      The attached email was provided to an employee who read it to ***** *******. Ms. ******* had me return after hours to tell me no that she would not do it. She refused to read the email or provide an explanation. Even after I reminded her that she insisted that the Social is written on the cards. The email indicates that she failed to enter the number, and she stated in her response that she did not complete the service

      The social security number is not required for live scan. However her particular live scan for teacher credentials does request to put a social security number on the form what she didn't do and because she didn't put it on the form I didn't put it on the system. Teachers credentials live scan instructs the applicant to include the social security number on the form unknown to the operator, therefore the operator its not responsible for entry in social security number if the applicant does not provide one.
