I am writing to formally dispute the medical **** I received, dated August 27, 2024, in the amount of $1211.00 addressed to ******** *****. This bill is not mine and is a result of identity theft. After reviewing the details of the bill, I called your company and was advised that the information on record was similar to my name and DOB. The spelling of the name you have on file is different from mine. Additionally, I learned that the ****** who received ambulance services was not able to provide any further details at the time of receiving medical attention including a home address. The services were provided to an address of ************************************** which was at ********, an auto parts store.After I asked three questions, your employee asked if I was claiming identity theft which I found odd as I have not once mentioned that, I was simply gathering information as to why I would receive a bill for someone else. I asked the employee how my name and address was gathered and she stated that Wittman Enterprises has a system that searches similar demographics and it was possible that my information was similar to that of the ******** that received treatment and sometimes the system will "match" the demographics to what they find in the system. Learning this is deeply concerning as your company has FAILED to diligently investigate the proper billing information for the correct ****** who needed medical assistance. The employee advised me to simply send the bill back saying it was not me or to write a letter with my photo to prove it was not me. The mere lack of concern to expedite and resolve this issue and simply "send back the bill" so the employee can flag for her manager is unacceptable and indicative that I am not the first ****** erroneously billed incorrectly.Business Response

I am rejecting this response because the company has failed to provide me with the following request in writing:
Immediate investigation into this matter to determine the origin of this bill and the legitimacy of the services rendered.
Correction or removal of this erroneous bill from my records.
Provision of all documents related to the medical services claimed, including but not limited to, copies of medical records, dates of services, and any signatures obtained at the time of service.
Notification to the credit bureaus that this debt is disputed due to suspected identity theft.The company did reach out apologizing and stating that they will send me a letter with the requested resolution of the erroneous bill but until that is received for documented verification of my records this is still an open concern for me as there is no proof that I am removed from this billing. Once the letter is received with documented notification that I am not being charged for a service I did not render in addition to ensuring that my personal information is not attached to an ficticious profile then I can accept the response.

Services.webillems.com billed Blueshield of Ca $500 for a date of service 3/19/24. My bill was denied by ********** of Ca because that is not my health insurance the date of incident. I received a mail about my bill being past due. I have called the ph# ************ but it is difficult for a worker to answer. I have submitted 2 inquires regarding my bill, but there is no response. They are not charging the correct billing department which is Western Health Advantage for the date of incident, but instead charge ***********, whom I never enrolled with ever.Initial Complaint

Wittman Enterprises LLC / WeBillEMS has been attempting to bill me for ambulance services rendered to someone in *********, ** in February 2024. I was not the individual who received these services and have not resided in the state of ********** for over a year and was not in ************* in February 2024. There was some kind of records mix-up.I have been in repeated contact with Wittman Enterprises and have repeatedly submitted documents (IDs, utility reports, etc.) at the request of Wittman but none will satisfy the agency. Date of service: 10/06/2022 Run Number: 22-629088 I keep receiving the invoice from the *************** *************************** (CAEAS) for $143.70 for my daughter, ************* ambulance service which my insurance (United HealthCare (UHC)) should be responsible for the billed. I have contacted my insurance UHC and UHC have made *** (direct deposit) payment ($143.70 plus $15 interest, total $158.70) to CAEAS on 02/27/2023 (attached *** deposit record). I have asked UHC to contact CAEAS numerous times but CAEAS keep sending me the invoice for $143.70. I also filed a case #******* with the ********** of ******************* (attached letter). It has been almost over 6 months, please check the record and find the *** payment deposit information and credit it to the account properly. The account should have a zero balance.Business Response

For over a year, I have been told that the check I mailed in January of 2022 for an ***************** Subscription was retuned and am being billed $25 for a returned check fee. I have provided them with all of my proof including the bank statement which shows that my check cleared in March of 2022, copies of the Electronic Endorsement provided by my bank which shows that the check was presented twice by them. After previously providing a copy of the page from my bank statement showing the check cleared, they insisted that I had to provide ALL of the pages of my bank statement even though it consisted of my debit card transactions which had nothing to do with my check payment. That was over a month ago and still after speaking to another representative today, I was told they needed more time to review. I have called about 10 times over this matter, and they still refuse to fix it. I fear what my experience will be if I ever actually need ambulance service from them.Business Response

As a representative of the *************** and its patients, one of our most important missions is to make sure that participating patients are eligible for their benefits associated with the ***************** Subscription. While this was the inaugural year for the program and there were surely lag communication issues associated with multiple parties involved in the rollout and the process, it is also clear that all applicants received the Subscription coverage they paid for, including **********************
We deposit all City funds directly to their bank account; however, there isnt any way for ** to re-deposit payments. We concur that the payment was received and deposited by us directly to the bank. Our team later received a bank notice indicating the check was to be referred to maker after we had deposited the check on behalf of the ***************. We dont have access to the ********** statement to review any refer to maker information, nor do we receive a patients full bank statement to reconcile.
For ************** desired resolution of his questions, we sent a packet via **** Certified Mail on Friday 6/9/23 including the following patients requests:
Provided a summary letter indicating that neither the City nor Wittman Enterprises disputed the fact the check was received and deposited; but, that the subsequent correspondences were the result of the information received directly from the bank;
Provided proof that the patients 2022 Subscription was indeed active;
Provide a zero balance invoice from 2022 reflecting the conclusion of the investigation; and,

Received a billing notice yesterday (4/3/23) from this company regarding a supposed charge that occurred 12/30/22. Why such a long delay? The "ambulance" bill occurred in a place I've never even been to. Obviously my personal information was compromised. When I called to find out why I got this notice, I was told to write a letter with my dispute. It is the year 2023, why can't the dispute be submitted digitally, especially when they request a copy of my ID. Smells like shady business practice to me.Business Response
