    I received a consultation to see if Dr. ********************** could fix the gap between two teeth (bonding). The assistant (Dr. **********************s wife) took me to the room, reviewed my forms, and asked me who my previous dentist was. I answered that the treatment was done in ************* and told her that I would like to not provide my dentists information because I felt it was unnecessary since it was just a consultation. She asked me a second time and I politely declined. She then asked me a third time which made me feel very uncomfortable and harassed. When I did not provide an answer, she rolled her eyes and gave a huge sigh in frustration. At that very moment, Dr. ********************** entered the room and spoke with me. We had a brief conversation and agreed that he could take new X-rays as needed and that I would come in again to have the teeth bonding procedure. He also assured me that his receptionist will provide me with a quote. The receptionist said that she will send it to me tomorrow morning. I never heard back from her, so I called the office the next day. The receptionist told me that after I left the dental office, Dr. ********************** made notes that he was not comfortable working with me and then she later said that Dr. ********************** was not confident in helping me because I did not provide any information about my orthodontist. I never had an orthodontist; I only worked with a dentist. Dr. ********************** did not follow through on what we mutually agreed on, and neither he nor his receptionist have the decency to inform me. They just decided to brush me off and hide. The work ethics in this office are questionable. Dr. ********************** does not follow through on his agreement. Dr. **********************s patient forms explicitly state that he and his team will not retaliate. However, Dr. ********************** and his team are retaliating against me by refusing to provide the service that was discussed and agreed to because I did not provide my dentists information that had nothing to do with the service that I requested.
