Our medical practice located in Brunswick ME 04011- received a fax attn to one of our providers regarding one of her patients. It was a request to sign a prescription for multiple orthosis products and to send it along with the last 1-3 months of office visit notes. The patients Medicare information was attached as well as her demographics information. The doctor did not order this prescription and she called the patient who said she knew nothing of this prescription but did say that she is getting multiple calls to her home about it. Our practice did not send any of this information outside of our office, nor did we order these products for our patient. It was brought to my attention that our medical records department received a phone call from this same person/company last week requesting this same information be sent, they were told we did not order these devices. The fax came from InCall Medical Supplies.. from Daisy Robenson... dated 10/28/2024 subject: urgent prescription. "The notes on fax: DO Request: As per the conversation with the patient we need to process the below orthosis products.
