Description
Received several telephone calls early October from an unidentified party (1-904-680-0103) requesting medical insurance information required by an unidentified primary care physician. I refused to provide any information. I assume the product received below was related to these calls soliciting additional information. On Thursday, 17 October 2024, I received a package containing a wrist brace, -L3916 Comfortland Premium Wrist/Hand Splint. I did not order this product. The supplier was AVA Med Supply, 321 Northlake Blvd, Suite #116, North Palm Beach, FL 33408, Order # AVA-46695. The bill to address was Delta Medical Group Inc., 7324 Southwest FWY, Ste 820, Houston, TX 77074, Invoice # 46695. I notified MEDICARE of the potential fraud/scam. I was advised to return the product to AVA Med Supply via returned receipt requested postal mail.
