I received a letter from a company, alleging to be Maximus, a CMS contractor knows as Maximus Federal Services, Inc. The letter is requesting my SSN, DOB, mailing address, etc. The letter does not provide any phone number, email, or website; only the PO Box. I called the real Maximus, which is an appeal company for people on Medicare to appeal charges on their insurance. The legitimate company informed me that this nothing from their agency, and that this is fraudulent.
