My husband received an EOB from our Medigap carrier than included four claims for a total of $9108 from Carebridge Innovations over a period from 4/2025 to 9/2025. After some online digging, we learned that this claim was for urinary catheters, which neither of us use. The Medigap carrier denied the claim because the vendor never submitted a W-9 form. However, we later learned that Medicare DID pay the claims and are currently attempting to get the funds back. (WHen reported to Medicare, the agent let us know that they have more than a few claims pending for fraud for this vendor.)
