Has individuals name and address on it requesting requesting to verify above information on letter . Bottom half of document looks like a check and has a place for a signature and it says on small print at the bottom that it is not a check although it may look like a check to an elderly person. At the top of the document that looks like a check it has a toll free number 1-888-801-8190 The document that has a place for signature at the bottom right hand side and a box above that with an amount that says allocated waiver.
