Description

I received Medicare summary of claims notice that included a $2,250 charge for 300 urinary catheters under billing code A4352-KX, billed by Carebridge Innovations, 5 times this year (April 22, May 22, June 20, September 4, and October 3). Medicare approved and paid some claims, total $3,958.56. I never needed, ordered, used or received such a product. Provider did not bill me for the portion that I am responsible for according to Medicare notice. I just called Medicare and reported this as a fraud Company information provided by Medicare explanation letter: Carebridge Innovations 1515 N University Dr, Ste 214, Coral Springs, FL 33071-6099 The. (954)828-0862 The claimed prescriber is Zinabu Maxwell; I never received treatment from a doctor with this name.
