Medicare and Blue Cross fraud Med/Surg Supplies were billed and paid by Medicare and Blue Cross Using my name, address, Medicare and Blue Cross numbers I did not receive any supplies or service from this company Reported to Blue Cross. Unable to report to Medicare with Gov. shutdown I live in Illinois and would not be receiving any service from Texas Reported Identity Theft to County Sheriff Attempted to contact GPAAK Equip Co by phone. Only get a busy signal.
