Description
This company continually is sending us an invoice asking for payment for "consulting". I am the Director of Operations of a very small company (5 people including myself) - we have not used this company for anything. I have told them multiple times and they just send another invoice. No phone number is available. I am attaching the W-9 they sent for me to set up payment.
