Annual records service 1030 15TH ST NW #366 Washington DC 20005-1503 Good thing I was looking up forms on the IRS website when I encountered this form and how it was a scam. They almost got us for 119$ the form looks legit but some of the wording seemed off to me . With that said something told me to look it up . I'm glad I did.
