This content is based on victim and potential victim accounts. Government agencies and legitimate business names and phone numbers are often used by scam artists to take advantage of people.

Description

This fraudulent claim appeared on a Medicare Summary Notice that was received by mail on July 23, 2025. The claim was submitted by Cambridge Innovations in Coral Springs, Florida for 300 intermittent urinary catheters with insertion supplies for a total of $2,295.00, none of which were ordered or needed. These supplies were supposedly ordered by William V. Jacobsen, a medical person/scammer at Cambridge Innovations. We have contacted Medicare and the secondary provider and hopefully with enough complaints, they will target these companies committing fraud and put them out of business for their illegal and appalling business practices.
