Primary Care Provider office has received numerous faxes and calls requesting patient records to authorize multiple patients to receive various medical braces (wrist, back). The most recent fax had a fake order with the forged signature of one of our providers on an order. Two patients, both Medicare, contacted us as they had received multiple calls from the company as well. Neither patient had any issues requiring the devices. The office received 2 calls today, from 2 different numbers, providing 2 different caller names; both calling from "Synergy Medical Supply"
