Fake government (Medicare Savings Program) letter claiming to be able to get recipients refunds for Part B premiums, Extra Help for drugs, and extra benefits for dental, vision, hearing, and transportation. The envelope says "Second Notice/Time Sensitive," "Important Document Enclosed" and the return address is "Records Division, P.O. Box 100090, Kennesaw, GA 30156." Inside is a notice made to look like it is a government form (has T-2 in upper left corner and 2025 in upper right corner) and is entitled "Medicare Savings Program." It instructs recipients to send in the inquiry card to see if they qualify for the above benefits and asks for personal information, such as name, age, address, phone, spouse's name and age. I believe it is a scam to get people to transfer from original Medicare into Advantage Plans promoted by private insurers, by making it look like an official government form that requires action immediately. In extremely small font that elderly people would have trouble reading (and located just before a bar code) it says: "Not affiliated with or endorsed by any government agency."
