Date of Transaction: 5/15/22 Amount $50 My daughter was in the facility on 5/15/22. I paid a $50 copy for services, however, my insurance paid at 100% and there is a credit of $50 owed to me. The office REFUSES to give me back my credit. They did acknowledge that there appears to be a credit on the account and they are "looking into it." It has been months and no action to return my funds to me.Initial Complaint

This complaint has to do with billing from a visit in March of 2020, for which we were billed incorrectly. I was billed for this ONE YEAR later, and was billed for a level four visit when strep throat is obviously a level one visit, no one was with us for more than fifteen minutes. This was resolved and a new **** sent and paid online on 6/7/21. On 6/29/21 I received a **** that said my account had been sent to collections, attempted to call the company at ************ and went directly to voicemail. I left a message and then decided to connect with you all and complain. Furthermore, the location we were at appears to be closed (see attachments).
