      My wife took me to the ** on 9-1-21 where I was admitted and hospitalized with covid induced pneumonia. I am a retired veteran and covered under the VA as 100% disabled. My wife followed the procedure with the VA in order for my ** visit and hospitalization to be covered. I can provide authorization numbers if needed. I received one **** (paper statement) in the mail and no phone calls. It then got turned over to a collection agency. It wasn't filled under any insurance. My wife called (5-6-22) the client number on the collection agency statement, which is different than the number on the one/only statement received from the hospital, after talking to the VA, to ask why the **** was never filled under insurance. She spoke with a representative named ****** She was told it was a **** from the doctor ******** at Midland Memorial who uses their name on the statements, and told it was too late to file. When she asked why it wasn't filled under insurance ***** told her they didn't have any of the insurance information and in the past tried to call to collect. When asked to verify the phone number they had the wrong number on file!! My wife explained they had the wrong number so there was no contact made other then the one statement we received, and the VA said it would be covered 100%. ***** said it was too late to file under insurance. My wife asked again why it wasn't filled if the hospital had the insurance information. She was told again they didn't have it and questioned why it didn't transfer over from hospital to a doctor's ***** She said it was two different bills. We did not know this until today. Midland Memorial should have given the insurance information to any and all third party companies affiliated with their services and hospital. I've got that **** for $607.00 and another from a Midland Pathologists for $83. Again, my wife called after I was admitted into the hospital and got prior authorization for my medical needs to be covered 100% by the VA.

      Midland Emergency Managers (MEM) is in receipt of the May 9th letter that you sent to ***************************, Midland Memorial Hospital. Midland Memorial Hospital confirmed the patient's insurance was filed correctly and forwarded the letter to MEM to review. We have completed our review.

      We determined that the original demographic information sent to MEM did not have the correct insurance information and the patient was registered with a financial class as self-pay. Three statements were sent to the patient before being referred to the collection agency. The patient's wife contacted us on May 6, 2022 and spoke with a self-pay representative. The account in question was recalled from collections. We have since corrected the insurance information and reprocessed the claim which was accepted by the payer on May 10, 2022. If the insurance processes as past the filing deadline the account will be adjusted off.

      We appreciate the opportunity to address the patient's concern. Please contact me directly should you or the patient have any questions or need further assistance.
