Bridge Solutions did not submit numerous documents to the client's insurance company. These documents were requested by both us and by the insurance company. The insurance company said it has been 21 days since their request was made. Furthermore, Bridge Solutions did not have the caregiver complete an accurate daily log. The caregiver merely checked everything that was part of a care plan at the end of the day. The caregiver did not do at least 50% of the things that were checked off. I asked Kenny at Bridge Solutions to correct it and sent it to us. She never did.
