On Monday, September 20th, I had a scheduled appointment with ******* to have blood taken to test my blood type, as well as for the COVID Antibodies test. I selected ******* for my lab work because they are "in-network" with my insurance company. When I arrived at *******, the representative collected my blood, but stated that I would need to order the test through "requestatest.com". She also stated that I would be reimbursed through my insurance company. I contacted my insurance provider and they gave me the following list of information needed in order for them to reimburse the $45 that I paid: * Diagnosis code * Description of services performed or a billing code * Date of service * Federal Tax Identification Number (from treating doctor or facility) * Billing and servicing address * Receipt showing payment I emailed "*********************" on 9/22/21 requesting this information so that I could be reimbursed. They responded on 9/22/21 and stated that they cannot provide the infoBusiness response

Better Business Bureau:
I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint. For your reference, details of the offer I reviewed appear below.
This is a blatant lie. I'm the one who has emailed and called. I spoke with a DIRECT REPRESENTATIVE of requestatest. There is no third party company. Why must you lie like this? I also have receipts indicating I've reached out to no avail.
