4/14/2022, I had my first appointment with ***************************. The intake paperwork that I submitted listed charges for New Patient Consultation with Treatment as $75. I paid $75 at time of service under the assumption that it would go toward services rendered and any overpayment after insurance paid I would receive back. When I received my EOB, I learned this provider was not in-network with my insurance. That was my error. I called *************************** office when I received a **** for $85. I explained I already paid $75, despite the statement not reflecting this, and would pay the additional $10. I was then informed that the $75 charge is separate from treatment and is a charge they assess for new clients and it is not a charge that insurance will pay for. No where in the paperwork, and never was it explained to me, that charges would be assessed that would not be billed to insurance. The paperwork I signed for them shows that the New Patient Consultation is "with Treatment". Also, my EOB shows two separate charges (CPT codes) for treatment, this is what equals the $85 charge they are billing me for. I was never informed, of and did not consent to, TWO separate charges for two treatments separate from the "New Patient Consultation with Treatment." I also think *************************** was pushy with trying to get me to purchase additional books, tests, and products as I was leaving. My appointment was rushed and was interrupted by another staff member telling *************************** that another patient was waiting for them.
