      This dentist overcharged me. The original amount was for 415 when I contacted united concordia dental. I have paid them 993 dollars and have been discriminated against by the office manager *****. I was told by my insurance as well as ***** that since services were not rendered I am *********** a full refund. I have been trying to get ***** to refund me since 12/17/2023. She is actively not calling me back even when staff members have told me they have escalated the situation with her. Not only that she was told by my insurance I need to be refunded but instead she walked away from me and ignored me. Which I have a witness to this whole interaction. At this point I am probably going to have to *** in order to get my money back since she is ignoring me on purpose at this point.

      Patient was seen on 8/14/23 for broken crown. Dentist remade broken crown and no charge and also identified a new tooth with decay to the nerve that required a root canal and crown. Doctor prepped tooth, scanned for crown and scheduled patient with endodontist for root canal treatment. When we prep the tooth for the crown and scan, we charge out for the crown. This is a standard practice to charge out on "prep date" by insurance guidelines. Once root canal treatment is completed and decay completely removed, we can permanently cement the crown. This patient canceled her root canal appointment 3 times and walked out on the fourth appointment without being seen/treated by specialist. Since necessary treatment patient was made aware of was never taken care of by her, we were unable to permanently cement her crown as we cannot permanently seal in active decay. If she receives the root canal on the tooth, we are happy to permanently cement the crown. But the work has already been done and crown made, hence what we charged her for. It was the patients decision to abandon without completion. We are happy to help her complete her treatment.

      On 12/29/2023 I made a follow up appointment to proceed with the treatment plan as discussed which was either traditional fillings on my teeth or crown essentially every tooth with a cavity on it which would be appprox 22k after insurance as opposed to $129 for the filling of cavities after insurance. When I arrived at the appointment the dentist refused to fill any cavities or treatment at all unless I agreed to all porcelain crowns. I was not given an explanation other than fillings are only temporary. Because they refused to fill my cavities and it was the Friday before the new year I was unable to get in with any doctor to get my fillings done as they scheduled the appointment for fillings and then suddenly refused when I got there. Dentist originally pushed for crowns however that was not necessary, it was only one of two options, so I elected for the lesser option. I was also told x-rays cannot tell depth of cavity. Im no dentist but I thought that was the whole point. Out of the probably 8+ cavities, they refused to treat a single one and said it was going to a lot of work for the dentist.

      Here is my visit summary from ANOTHER Modern Dentisty branch in ********, ** who completed a filling on one of my cavities that Davenport Modern Dentistry refused to treat.  This was one of my front top teeth so I did not want to smile or talk to anyone because of embarrassment of having this noticeable brown cavity on a front tooth. I went thru Christmas and New Years without access to another dentist because it was a holiday weekend so I was completely attending Christmas and New Years events completely humiliated because Davenport Modern Dentisty/Dentist decided without notice that they were refusing to fill the cavity and unless I opted for a porcelain crown which was $661 out of pocket after insurance, they would not treat me. This also could have been put towards last years 2K dental benefits under insurance but now went toward this years 2K maximum benefits so I have less benefits to utilize this year and have much more dental work needed.

      I just received this bill/explanation of review for an office call that never occurred. They refused treatment all together. There was no call whatsoever as they turned me away when I got there. I spoke with the dental office as I saw this on my delta profile prior to receiving this correspondence and the office manager told me nothing was billed for that date of service even though I told her I saw that $49 was improperly billed to insurance as no treatment ever occurred. The office manager clearly lied to me as I see the non-visit/non call did indeed get submitted to insurance for payment. This is fraudulent billing.

      3/31/2022 Root canal was performed by ******************* placed by ************************************** 08/30/2022 went back in due to uneven bite. At this time, I had major gum swelling, however the dentist **************** did not treat the gum; only treated the uneven bite. Xrays were done this day to examine the tooth. 09/08/2022 Went back in due to continuous gum swelling in same area where crown was placed. Another xray was done and both ******************************************* then said there was a major infection and that the tooth needed to be extracted. They said I could come that sunday morning to have it done in office. I was very upset that I was there the week prior when xrays were done on that same tooth with the same swelling, and no dentist mentioned infection. After this, neither dentist ordered or offered antibiotics for me. Instead, they said they could refund my money only if I wanted to put that money towards another procedure with them. I stated I did not want to have any more work done here and that I just want the refund, however the manager ******* declined this. My mouth ended up abscessing from the poor dental work that was done and their inability to treat my infection. I was sent into surgery to have the crown removed, tooth extracted and infection cleaned out. At Davenport Modern Dentistry, I was charged 1,628.28$ for the crown, as well as 36.68$ for the visit on 09/08/2022.
