I am in urgent need of copies of my EOB's plus any other claims or records available.Since at least Feb 10 2022, I have been contacting MHN to get same.They replied 2/10 and confirned such would be emailed.On 2/16 I emailed them a second time, and no reply was received.On 2/18, I called and spent half an hour of my day trying to follow up on this simple request.***** in claims assured me she woud, email them in 45 minutes and call me back to confirm receipt, this never occured and now they are not replying again.This documenation is NOT available on their site.Very poor customer serivce.Initial Complaint

I received a notice in the mail one week ago (the envelope was postmarked on 8.27.2021) for services that this company is saying that we have been billed for. I have never been to this healthcare provider and I called them and they confirmed this. Someone is using my information to wrongly acquire treatment and I am being billed for it. It says in the paperwork inside the envelope that "this is not a bill" but it's a statement of services rendered, the healthcare provider that rendered the services, and the amounts that we were billed or would be billed. The supposed Provider is "Pacific Coast Psychiatric Associates" that is now named Lifestance. I've filed a grievance and they have not taken any action. Furthermore, I have contacted Health Net that is supposed to be the mother company and filed a report but they said an update is indefinite. This is absurd since it's my personal information that has been used to commit fraud.
