This plase needs to be investigated for stealing peoples medication! So many lies about saying they gave pain meds when they didnt then getting mad when confronted. Talking openly about people in the hall ways using first and last names talking about medication they cant find for them, Hello HIPAA!! Staff is so rude, calling people bad names when they get mad. Not providing medication on time and making people wait hours for it. CNA will be nice to your face then treat your family bad as soon as you leave. The room was always so dirty. So many things I wish I could remeber it all. I think someone needs to take action and shut this place down. Stop letting staff steal drugs from the people in pain that need it!!

The **** ******************************* is the worst Supervisor I have ever worked for. I wasnt even there a month and she accused me of all sorts of things that never happened nor I did.

My mother was moved to this facility. Bad move. She was constantly left in her own f**** for hours and or next day. Nurses were rude and unprofessional. There is no availability to talk to anyone there as far as Doctors or *********************** They DO NOT return phone calls nor communicate. The place is a horrible example of where to put your loved one for care. They get sent there to die. They did NOT get any help to rehabilitate her arm after a stroke and now it is beyond repair. DO NOT put your loved one in this facility. Extremely short staffed.

If I had to give a star it would be zero....this place is so short staffed 1 Nurse and 1 Medical Assistant for 30 patients per section....one of the nurses very rude and sarcastic ...My dad was sent there for rehab and he fell on Friday I found him lying on the floor in the bathroom, I'm not sure how long he had been there, if I had not gotten there who knows how long he would have been there. On Saturday he had been calling for assistance because had had had an bowel movement accident, when i got there he told me he had been ringing for over 20 minutes and no one had come to assist...I went to search for someone to ask why no one was available and asked them to just give me what he needed, they said they would send someone in 40 minutes...i told them 40 minutes would you like to sit in a f**** for 40 told them never mind I will help him if you don't have time to assist him right now...then they said we will send someone right now...when the assistant came I told her about the incident on Friday...She said they have a schedule to check on each patient every 4 hrs...Can you imagine my dad lying of the floor for 4HOURS or even sitting there waiting 40 minutes in f**** to be changed....I have been there calling for assistance for my dad myself and I ring and ring for a nurse and no one comes until i have to go search for someone and even then, I can't find anyone!!! This place is full of patients and not enough medical staff to attend to them...another thing when I call for an update on my dad no one ever answers the phone. Patients are very neglected ignored don't sent your loved ones there it just horrible place.

The facility was run in an intimidating manner. They tried to intimidate the patients and family members. My experience was absolutely horrible. I was completely coherent and mentally able to process what was happening but my roommates were in their seventies and very vulnerable. I documented about 19 pages of incidents. The facility did call me and asked me to do a survey. I was honest! No one seemed to know what they were doing. The OT/PT staff were very good! One nurse named Apple was very good! Everyone else was really poorly trained. Ive cold food! Overdosed me on my insulin 3 times. I rarely saw a doctor. It was unbelievable! Like no one was providing good training for the staff. Our bathroom wasnt cleaned for 3 days! Our meds were never given on time. Our window was damaged so anyone with a screen cutter could had crawled right in. We were right next to the parking lot. We were given a shower with lots of residents, no privacy and male staff. It was disgusting! I told them to leave! It was so inappropriate! Patients were sitting half way in and half way out ou the shower in their wheelchairs. Just horrible conditions! Dont send your loved ones to this facility! It looks like a hotel from the outside but its a h*** hole.

This place is similar to a **** camp it should be called (Peoria post acute concentration camp )The patients there living miserably and been neglected , left alone for a long period of time soaking wet in their urin and f**** they are been ignored neglected and degraded when you try to complain to the nurses or their superiors they get defensive defiant and rude They have the attitude of we dont give a ****, we dont care and there is nothing you can do about it When did it became ok or acceptable for a place that supposed to be a home for caring rehabilitation getting better and hope to Become the very source of uncaring ,aggravation ,agony ,distress ,cruelty, and heartless and that what patients go through at the Peoria post acute and rehabilitation.

No one answer the phone at night. Wanted to check in my sister she was having chest pains and they wouldn't call a medic rescue to take her to hospital and Im worried.
