Late in the day On Thursday, November 18, I received a phone call from a Faviola Paz, who represented herself as an Account Manager for a credit card clearinghouse. She is located in Encino, California. An on-line Pre-Application was filled out, and submitted to her company. She said that it looked to be fraudulent, and she said that all of the material sent to her would be forwarded to me in the morning. Faviola asked where the man was calling from when he called to see if the information was received. He said that he was in Ohio. Faviola told the caller that her phone displayed a Florida area code. The man became belligerent at that point, and she disconnected the call. She told me to feel free to contact this individual, but I don't know who to believe at this point. Faviola told me to fill out a police report, which I did last Wednesday, with the Toledo Police Department. Some of the information on this Online Pre-Application is correct, and a lot of it is not. Not much is correct on the driver's license, and nothing is correct on the check--except our name and address.
