Description
CAREBRIDGE INNOVATIONS LLC submitted a claim to Medicare and my supplemental insurance for $2,295.00 claiming I ordered 300 catheters. I do not use catheters. They listed an ordering/prescribing doctor I never heard of who lives in another city in my state. I never received any items from Carebridge Innovations LLC. This bill was paid by Medicare and my supplemental insurance company before I received my EOB. As soon as I discovered the fraud, I reported the fraud to Medicare and to my supplemental insurance company. Subsequently, Carebridge Innovations LLC submitted 5 additional fraud orders to my supplemental insurance company for a total of $9,180.00. The supplemental insurance company did not pay. Total fraudulent charges to date are $11,475.00.
