Genetic kit comes from Living Well Screening 1007 Mantua Pike Unit 101A West Deptford, NJ 08096.(reported as scam already) Paperwork has Dr Barbara Bellar npi 1447309208 real per NPPI site. Located in IL. Called given number a person answered stating to be Dr Bellar. I know no doctor that answers their on phone. Gave me the name of Dr Ford owner of CTD Network. Located in Plant City, FL. Provider information 813-807-3338. 1805 James L Redman Parkway suite 201 Plant City, FL 33566. TOO MANY DIFFERENT PARTS.....Paperwork already has Medicare number on form. Dr. Bellar stated its optional and Medicare is now paying for the genetic testing.
