Service Date: 11/3/2023 Amount Owed: $1000.89 What is the nature of the dispute: The business incorrectly billed the insurance provider, so that they could deny the claim and have me pay the full amount. On 1/25/2024, I reached out to the business to discuss the charges, and to ask why I was responsible for the full amount. They advised me that the insurance didn't cover the charges because the service was out-of-network. Additionally, the representative told me that they had to process the claim as if it was in Florida because the requested physician was licensed in Florida. I don't even know why they bill the **** OF FLORIDA, when the service was provided in Maryland. They are avoiding to receive the agreed amount from the insurance company; instead, they want me to pay $1000.89 for lab work. This is completely unacceptable, immoral and unethical. On 1/25/2024, I reached out to the insurance company (********* **********), and they advised me that Quest Diagnostics incorrectly submitted the claim. I was told that Quest Diagnostics use the incorrect NPI number, one for Illinois when the service/specimens were provided in Maryland. They told me to reach out to them and advise them to process the bill using the correct NPI Number for Maryland, which is where the service was provided so that they could approve the claim.
