    I saw Dr. ********************** on September 22, 2021 to inquire about ********************* services. I filled out intake forms through his online portal. When I arrived for my appt, however, I was told there was an additional intake form I was supposed to sign in the office before I could see the doctor. A few weeks later, I received a call from his office saying that my vials were ready. Apparently the form I signed in his office before I saw the physician was a consent form for allergy immunotherapy services. Dr. **********************'s office told me I am liable to pay for the vials regardless of whether or not I would be administered the shots and soon after I am receiving a bill for >$1000.I felt misled and asked the office to send me the consent form so I could see what I signed. The consent form included 4 pages. I now saw that one page contained info about how the initial charges for immunotherapy include the preparation of several vials-- hence explaining why I got 4 charges. However, I never saw this page, and it didn't have my initials like every other page. The staff presented this consent form to me before I ever saw the physician and even learned what ********************* was.On the last page, Dr. ********************** signed next to a statement stating that he informed me about the details of that consent form and the services included in it. However, this statement is untrue because I was given that form and told I needed to sign it before I could see Dr. **********************. We never talked about it. I was presented that form as an intake form necessary to complete before I could see the doctor. I didn't think anything of it since I did most of my intake forms online and was led to believe that they just forgot to upload this form to the online portal. ********************* entails receiving a shot every week for 3-5 years and I should not have been given a consent form for said service before I ever even had a chance to speak to the doctor and learn more about it. And now, they say I owe $1009.
