Date of Transaction 5/10/2022 Amount of Money $3723.00 Visiting Angels Caregivers were to care for me (24 hrs,) during the Post-Op week (Surgery 5/9/2022 Major Surgery Total Hip Replacement). The caregivers failed to provide, proper assistance as outlined in the Plan of Care Document. They were, rude, lazy, did very little in the day shift (12 hrs), Night Shift were sleeping (12 hrs.) One injured me and another I caught,sneaking out before end of night shift (Safety Hazard). When I brought these issues to the Coordinator, ***************************, Supervisor ******* and Director Kit, they ignored my concerns. My daughter had to take a week off from work and come into town to help me with these issues. My credit card was charged $3723.00 on 5/19/2022. I received invoices for charges via US Mail on 5/24/2022, because I insisted on seeing what I was being charged for. To date, I have only been reimbursed for $1224.00 and the remaining balance is in dispute. I should not have to pay for services I did not receive. They caused me a lot of pain and suffering. The week of Post-Op surgery the Coordinator showed up at my home at 7:50am in the morning while I was still in bed. She had me sign forms, which I did not get a copy. I spent time trying to get copies of these forms, when I did receive them it had information listed that I did not list. Falsifying information. They took advantage of a disabled person.
