  Dear Better Business Bureau,I am submitting a formal complaint regarding the practices of a rehabilitation and nursing facility located in the *****, **. I will not include personal medical information or patient names in order to respect privacy regulations, but I want to bring to your attention a serious concern regarding the care and treatment provided by this facility.A close family member of mine was admitted to this facility after leaving a hospital with clear medical documentation stating that the individual was not able to make independent medical decisions. Despite this, the facility required the patient to sign a stack of legal documents, including paperwork granting the facility power of attorney. This occurred while the patient was in a state of delirium and documented by a physician as incapable of making such decisions.During a family meeting with a doctor, we were told explicitly that this individual could not be released from care due to the severity of their medical condition. However, the facility later informed us that they would be discharging the patient despite those medical directives.We have also experienced the following issues:The assigned social worker has been unresponsive to repeated phone calls and voicemails.Instead of addressing family concerns, the social worker has made inappropriate comments to the patient, framing family members as the reason for continued care.The facility was originally presented to us as one of the best in the region for hospice and rehabilitation care, yet the experience has been the complete opposite.We request that the Better Business Bureau review this facilitys practices to ensure that other families do not go through the same distressing experience.Thank you for your attention to this matter.

  This place is HORRIBLE!!! The nurses, if you can find one are very nosey and unprofessional. They have absolutely no decorum. When asked to speak to the doctor about my father's health condition, he's never around. There's NO communication between the doctor and the family. THEY NEVER PICK UP THE PHONE!!!No matter what time of day. We've witnessed, firsthand, how when their phone rings, they just ignore it because they're too busy watching TV with the patients in the dayroom.This place is absolutely awful!!!It has a nice esthetic, but not much else.I had bought my father some clothes to have while he was there and, it was stolen from ****** father, who's diabetic, had his big toenail ripped. Literally ripped.Mind you, my father's always had healthy nails as my mother took meticulous care of his feet.They denied my father proper medical care after he suffered a slipped and ******* father was complaining about pain on his shoulder for weeks. I had to call the ambulance to take him to the hospital to get it checked because they wouldn't help him.This place is a full throttle ghetto.I wouldn't recommend this place to a dog.

  The Plaza Rehab and Nursing Center SHOULD BE SHUT DOWN,,,, MY MOTHER NEVER RECEIVED PHYSICAL THERAPY BUT THEY LIED AND CHARGED HER INSURANCE COMPANY. MY MOTHER ROOM WAS BY THE BACK DOOR WHERE VISITORS WOULD JUST COME FROM THE BACK DOOR AND WALK RIGHT INTO MY MOTHERS ROOM, WHICH WAS EXTREMELY DANGEROUS. THE ***** JUST IGNORES YOU. HUMAN F**** WAS ON THE ***** WHEN I COMPLAINT THEY SAID THAT THEY WOULD SEND SOMEONE UP TO CLEAN AND NEVER DID. ******* WOULD CRAWL OUT FROM MY MOTHER HOSPITAL DRAW AND WHEN I COMPLAINED THEY INFORMED ME THAT A OUTSIDE CONTRACTOR WAS HIRED TO HANDLE THE SITUATION. BED PANS WERE NOT BEING EMPTY SMELLING UP THE HOSPITAL. MY MOTHER COMPLAINED ABOUT A MICE IN HER ROOM AND I THOUGHT THAT SHE WAS BUGGING OUT INTIL I SAW THE MICE WITH MY OWN EYES RUN BEHIND HER HOSPITAL DRESSER. MY MOTHER WENT DAYS WITHOUT BEING WASHED AND THE FOOD WAS ALWAYS COLD. WHEN I ADDRESSED THESES ISSUES I WAS TOLD THAT THE SITUATION IS HAPPENING BECAUSE THEY ARE SHORT *****. THE PLAZA REHAB NEEDS TO BE INVESTIGATED AND SHUT DOWN. NO HUMAN BEING SHOULD BE TREATED LIKE THAT. I ALSO HAVE PICTURES. THE DIRECTOR OF THE PLAZA IS EXTREMELY DISMISSIVE AND RUDE. I WISH THAT I CAN LEAVE PICTURES. They do not deserve 1 star HORRIBLE

  This place is not worth of a subway rat. The incompetence almost killed my mother. The nurses disappear for hours and just don't care. My mother had pneumonia, they just let her lay there and suffer. Don't send your loved one here
