    In 2021 I required physical therapy and while searching for rehab facilities, I found Trinity Rehab. They had recently opened a new location in my area, however I learned they were out of my insurance network. Treatment was prescribed by my physician and was covered 100% by my insurance for in-network providers, including a zero copayment for PT, so I planned to go elsewhere. The director of this local facility met with me personally and agreed to treat me as in-network since they were just getting established/wanted to build their client base at the new location. She agreed to treat me per my insurance coverage, waiving any and all copayments. In March 2021, I signed a contract "which entitles patient to a 100% discount." It also states "patient responsibility is $0.00". There was no question between this local Trinity Rehab and me that I would owe nothing for their services.Long after I completed therapy with Trinity, they contacted me about payments owed. Although the director was no longer with Trinity, I forwarded my signed contract to Trinity HQ.I have spend hours on the telephone with Trinity HQ and the insurance company (sometimes at the same time so they could speak to one another in my presence) and each time Trinity finally agrees to drop this. On June 26, 2024 I received yet another email from Trinity Rehab about an outstanding balance for their services from 2021. Please make it stop!!!
