    Ive been wearing contacts since I was 16. I'm now 70 and I've had United Healthcare for Many years. I changed my vision provider and made an appt with Vision source. Now I've NEVER had to pay anything for my vision tests and contacts for many years. United Healthcare has always given me benefits to cover my needs. Well I was shown in and I was pleased with their professionalism. It seemed like I was being put thru a lot more tests than I've been thru before but I just followed their directions. As I went to leave and was asking about ordering contacts when I was given an invoice. The invoice said that I owed $99 out of pocket. WHAT!! We'll these tests they performed on me was outside of what is normally covered. I've never had them ANYWHERE ELSE. It could easily have been $500 for some test I'd be responsible for. I find that highly unethical. THEY KNOW BETTER! I DIDNT

    To Whom it may concern,

    Patient ***** ***** came into the office November 6, 2024 and had her exam. When she came to the check out we explained that her insurance does have copays that applies to her contact lens fitting and that we also charge a fee of $35 for retinal imaging that is part of our standard of care. She then stated that she has never had to pay copays in 17 years that she has had united healthcare. She was given the information to give to her insurance company to because she was going to call and discuss her coverage and stated we could bill her if she owed anything. 

    We sent her a statement 12/20/2025 and again 1/23/2025 with no response or payment. 

    Patient called 5/20/2025 with a representative of her insurance company on the line and wanted an explination if her charges and we explained to her that her insurance has two different types of contact lens fitting fees one is selection and one is non-selection. If the parameters of the patient's prescription fall into the selection category then the benefits do cover 100% of the fitting fee and up to 6 or 12 boxes of contacts depending on modality. However, if the patient's parameters to not meet the selection criteria then the patient is responsible for 100% of the fitting fee. Ms. Black's prescription requires both a toric or astigmatism and multifocal contact lens and there is not a toric or multifocal contact lens option on the UHC/spectera formulary list for the selection list. Therefore, the parameters for her prescription do not fall into the selection fitting and the patient is responsible for 100% of the fitting fee. She then stated that she should have been made aware of the fees before she was seen but she was a new patient to our office and without her seeing the doctor to determine her specific needs we would not have know if she would or would not have had selection vs non selection contacts until the doctor has prescribed her contact lenses. 

    We have attached documentation to show what her benefits state. Our multifocal fitting is and $80 charge and we give patients who do not have a benefit allowance a 20% disount as a courtesy to the patient already. Since she is unhappy with our services we will remove her fees as she requested.  

    We strive to give a complete healthy vision exam with our patients health at the forefront of our care.

    My complaint is simple. I was charged for something I was not aware of the extra charges. They easily could have added $500 as a charge for something. It is well known that Dr's and hospitals verify if a charge for a procedure was needed and the patients ability to have it covered with their insurance or private means BEFORE they proceed!
