      On January 2, 2024, I called the company to dispute a $41.40 bill. They told me the process, which is I can send them a written dispute and they would review it. So, I wrote the written dispute and emailed it to them that day. Now 7 months later, I have followed up with them over 25 times via email and phone. Every time I get ahold of them, they say the account management team will get back to you. I have not heard from them once in 7 months. As a result, this has incurred immense toll on my mental health. This has caused an incredible amount of stress over 7 months. This is extremely disappointing for a company to have business practices it does not follow. During this time, they moved my account to a collection agency, and I have had to explain to them multiple times of the situation.

      This matter has already been addressed by ***** *****, MBA, FACMPE, Chief Operating Officer of Capital Anesthesia Partners.  See attached email correspondence that I had with the patient on August 7, 2024.  Additionally, attached is the anesthesia consent form signed by the patient.

      In short, (1) the referring provider advised him that anesthesia would be necessary for his procedure, (2) the patient signed an anesthesia consent form authorizing care from Capital Anesthesia Partners and allowing CAP to bill his insurance, and (3) the claim was properly submitted and adjudicated by his carrier correctly.  The balance due is an accurate reflection of our work and his contract for insurance with ***** as well as our contract to provide services to ***** members.  Additionally, we are not able to adjust balances for co-insurance, deductible or co-payment per our contract with ******

      This matter has been referred to our third party collections agency.  Payment can be made directly to them by contacting ************.
