      From ********* I lived in *******, *******, and was a ******* *** *** ***** ****** ***** ** *********. Our Insurance company, **** ***** **** ****** of *******, was contracted with ********* Urgent Care in *******, *******, to provide urgent care services. ********* Urgent Care was eventually purchased by Fastrack Immediate Care. I patronized ********* Urgent Care from 2012-2018 and always paid my deductible, which is documented through my former insurance company. In July ****, I moved to *********, *********, but maintained a Post Office Box in *******, *******. On June 6th, 2021, I received a bill from Fastrack Immediate Care at my ******* post office box seeking $1162.31 from doctor visits in 2014, 2015, and 2016. I called my former insurance company, **** ***** **** ****** of *******, and spoke to a representative who contacted Fasttrack Immediate Care and provided them with proof of payment by both the insurance company and myself. The representative told me that the situation had been cleared up and commented that this was a widespread occurrence with Fasttrack Immediate Care. Beginning in June 2022, I received a bill from Fasttrack Immediate Care at my ******* P.O. Box seeking $218.32 for a doctor's visit in 2014. I decided to reach out to the BBB at the insistence of a former colleague who still lives in *******. We are soon to be nine years removed from 2014, and I am still getting bills for services that were already paid for by the insurer and myself. I cannot seem to get Fasttrack Immediate Care on the telephone, so this is perhaps the best way to secure a resolution.

      I went for a ***** ** test and was charged $*****.00 I have been calling for days Noone answers the phone..my insurance paid the 75 for the test but the remaining $*****.10 was the doctors fee for administering the test. Total scam. I will be contacting the local news saltation and I will stand in their parking lot and protest this business. They are scamming people. I was told I had no copay or deductible when checking out. Also the lady introduced herself as a doctor and she's not, she's a nurse practitioner. Which is fraud.

      **. ********, these are just fees that our set by our owners. I can assure you that you will not be responsible for anymore that what your insurance allows us to bill you for. This amount will be set by your insurance company. I will check your chart and will speak with the provider you seen that day.

      How can you look into my chart when you don't know the difference between are and our? Or that and than. Your lack of understanding of basic English doesn't give me any reassurance of your ability to problem solve or use critical thinking to resolve this major billing mistake.The provider is licensed as an anesthesiologist not a NP; we looked her credentials up. Also, this was billed as a visit to an anesthesiologist office in Washington and it wasn't. It was an urgent care visit to get tested for *****. That's illegal. The bill was submitted as an office visit to an anesthesiologist dynamite located in Washington and I have never stepped foot in an office called anesthesiologist dynamite. This was an urgent care visit and not billed as such. That in insurance fraud. YOU cannot bill for services not rendered and also lie about the location in which they were rendered to avoid taxes. I've done my research on the entire bill and those who submitted it. Copies of my findings have been sent to the attorney General for the state of Georgia.

      The provider you saw at our clinic is a Nurse Practitioner and has been for a long time. Your visit wasn't billed to an anesthesiologist office. We are under an IPA so with an IPA you use the IPA's insurance contracts which is what was done for your services. The visit was sent in under the location for the Albany clinic. This is not insurance fraud by no means since we are a part of this IPA.

      This is incorrect. The nurse that we were billed for has a provider ID. That provider ID is for an anesthesiologist out of Washington. My wife works for a medical billing company and looked it up herself. The provider ID that was billed does not match the location I visited. $4,000 for administering a covid test is unethical and has been reported to the Georgia ******** *******.

      You did see a nurse practitioner that day. As I previously stated we are a part of an IPA so we use all of there contracts and Id numbers but the location was Albany and the name on the claim is the nurse practitioner you saw that day.
