      I am writing to file a complaint against Dr. ********************** ********************* MD, concerning an inappropriate charge made to my account for medical services that were never rendered. I believe this constitutes a serious breach of ethical billing practices and customer trust.Incident Details:Providers Name: Dr. ********************** ********************* MD Amount Charged: $1,176 Date of Charge: [Insert date of the charge if known, or approximate period]Services ***************** visit charge Description of the Issue:I was billed $1,176 by Dr. ********************** ********************* MD, supposedly for a hospital visit. However, I have never received any medical treatment or consultation from Dr. ********************* nor have I ever seen him at any hospital as claimed in the billing statement. This charge appeared without my prior knowledge or consent, and no services were ever rendered to me by Dr. ************************* discovering this charge, I immediately contacted Dr. ********************* billing department to clarify and dispute the charge. Despite multiple communications, the issue remains unresolved, and the charge has not been retracted. The lack of resolution and accountability from Dr. ********************* office is concerning and unacceptable.

      Total Balance Due: $1,176.00 with four days of service
      This patient was provided four dates of medical services after he consented to be treated in ER. patient is only treated by any hospital physicians after they consent to be treated only  this patient was treated like any other patient and was billed for services and only allowed amount was expected to be paid. the above patient not only didnt respond  to the bills , call or statement, but after 4 years started to say that services were not provided to him.

      This patient not only is deliquent but also now falsifying information. please see below the details that hasan ********************** MD office and billing company tried ,  every thing  about his correspondence details  are in EMR notes .

      this patient was self-pay at that time of service , so balance was transferred to the patient as patient responsibility, as there was no insurance at the time listed or available. About the unpaid dues, billing  company contacted the patient on August 5, 2020 and September 19, 2020, but he did not answer the phone, even after  that  a detailed voicemail  was left . he was  also mailed  patient statements at his postal address, with the most recent one arriving on August 11, 2020. he never inquired or ask or bother to call back.

      Consequently,the account was forwarded to a collection agency.

      Nearly two years later, on June 29, 2022, the patient called us to ask about five bills that had been sent to collections. We made it clear that the hospital visits were associated with four bills, with four  dates of sevice.  We also told him about our earlier attempts to get in touch with him that were unsuccessful. The patient said he was admitted to the hospital during that time and thought charity should pay for the bills. We updated his contact information for future correspondence, and explained to him that hospital does not  pay  for physician services 

      On 07/14/2022, the patient  reached out again with concerns about the bills sent to collections. He was under the impression that there were five bills, one with a charge of $1176.00, and the others associated with Dr. ******************** Shahab. We confirmed with the collection agency that $1176.00 represented the total balance and the other four entries were the dates of service for  four days . The collection agency reissued the bill for the patient's convenience.

      After some more time passed with no word, the patient called on February 23, 2024,claiming to have been covered by ************************** at the time of service. we  are unable to handle it because we have lost the time filing limit to bill insurance
      To date, the patient has not indicated any lack of recognition of Dr. ******************** Shahab's services, nor has he disputed the legitimacy of the bills issued. Dr ********************** provided services to patient four times not one time during his stay in 2020.

      please review he first try to say write off in charity,  then provided insurance infornation after four years which cannot be billed , and finally falsifying that the services were not provided to him.

      He is not only paying the bills for services provided, but also complainiimg and falsifying the information

      ******************** ********************** MD was not even reimbursed for the services that were  provided on the contrary he is writing reviews on ****** and now complaining BBB.

      Based on his comments, the allegations made against Dr. ******************** Shahab were wholly baseless, and the patient is responsible   for paying the bills for the services provided to him
