    On 04/25/22, voicemail message received from Marymount Villa nurse stating the following: Mother was injured between 5AM-6AM in the bathroom, became agitated upon standing, lost balance, fell & hit her head on metal bar on left side of toilet creating a large bump on left side of forehead, scheduled for non-emergency transport at 2 PM Call returned instructing to call 911 immediately for emergency transport Took pictures of injuries: major bruising & swelling to face and shoulders. She should not have fallen with force to cause injuries when she was with two caregivers Staff did not follow protocol: give first aid & call for hospital emergency transport She is on blood *************** did not recognize danger of brain bleed *********************, Marymount Villa Director, was vague or just refused to give specific details Lack of proper care:Not wearing doctor prescribed compression stockings or diabetic shoes, visible signs of swollen ************** gauze taped to feet, open sores, blisters, broken toenails, healed wounds from wearing improper shoes.Not dressed for weather conditions, not wearing coat or bra, wearing dirty clothes, hair uncombed. On 6/29/22, mother was seated & eating a bowl of moldy grapes. Brought to attention of ***** (male caregiver) Condition of majority of clothes/personal affects: shrunk beyond wear, torn & missing Critical medications not filled for over 6 months, missing from inventory & missing stockings Since ********************* has taken over management, observed significant decrease in the quality of care: ************************************************* of residents Decrease in quality of food Inadequately trained staff This all lead to a tipping point to where my mother was injured. During the time of COVID, I was forced to find new housing for my mother. I am writing this complaint to warn potential & established residents that this facility is negligent in providing a safe place for elders.

    Price ******** ********************** in ************* Facilities My mother has been a resident of Marymount Villa (MV), an assisted living facility for almost 10 yrs. Executive Director ********************* requested several rent increases from $3,200 to $4,800 within a three month period. Due to the fact that my mother is elderly, disabled, suffers from dementia, she is conserved under the ******* Co. *******************.To validate any rental rate increase, the ******************* required medical justifications documented by medical assessments from MVs *********** mothers primary care physician (***). The nurses assessment put my mother at the highest tier (that of a bed-ridden resident) & the ***s assessment reported that my mother was not at the highest level of care. ***** alleged that the *** falsified the assessment & accused the family of influencing the doctor's *********** The conservator did not authorize the rent increase & continued to pay the prior agreed upon rent. ***** accepted all payments & cashed all checks, however ***** kept a running tab of what she thought was delinquent payments due (up to $10,000). She then stopped sending the rent **** to the conservator & started sending the bills to me in an attempt to circumvent the conservator. The conservator reported this behavior to the Alameda County Ombudsman.***** refused to negotiate the rent & stated if the family was not happy with the rent increase that the family could move our mother out. It was suggested that ***** could pay for her own independent medical ****************** scoffed at that. Now the quality of care has decreased & my mother fell & hit her head while with 2 caregivers.MV uses threats of eviction to push out residents that are not paying top dollar. The rules for assisted living allows for rent increases but to increase $1,000 + is exorbitant & unattainable, forcing those who cannot pay to leave. This is a money grab at the expense of our seniors.
