    I have been a patient of the therapy institute since and a great clinical fit since 2021 and have a complaint against Midwestern University. I believe that the university has violated my patient rights, engaged in misconduct, and failed to provide a proper platform for voicing my concerns. I kindly request a comprehensive review and appropriate resolution to rectify the situation.Waitlist Placement: I request immediate placement on the waitlist for services at Midwestern University. It is essential that I receive the appropriate care and support provided by the university. Redaction of Unfair Notes: I ask for a thorough review of my medical records and the redaction of any unfair or untruthful information that does not accurately represent my situation. It is crucial to ensure that my records are based solely on factual information.Closure and Goodbye to Former Therapist: As part of the resolution, I would like the opportunity to properly say goodbye and have a closure session with my former therapist. This would greatly contribute to my emotional well-being and provide a more positive transition.Violation of Patient Rights: Midwestern University, specifically ******************, ********************, and ******************, has violated my patient rights, potentially tampered with my medical file, and subjected me to verbal and emotional abuse. These actions have created a hostile environment and compromised my well-being.Misinterpretation and ******************************* has misconstrued my communication and actions, unfairly criticized my thank you email to my former therapist, and wrongfully terminated and retaliated against me. I never terminated myself This has further exacerbated the situation and hindered a fair resolution.Denial of Appeal Rights: Midwestern University denied me the right to appeal decisions made by the clinic, depriving me of my patient rights. I urge the clinic to establish a clear and transparent appeals process to address concerns and disputes.

    I had an issue with my front teeth from the first visit years ago. (When they said I could no longer be a patient, before I got home they had already disabled my online account with dates and costs). I was never told that the issue would continue to worsen (I had resolved the issue that caused the problem in the first place). I went there at least every 6 months for other dental issues that the worked on, never saying it was getting worse. I would have addressed that situation as my top priority. When I noticed the issue, I went in for advice. The student with the advisor said they could try a procedure that would make all of the dental issues go away. It needed to be approved by Grand Rounds but it had a 90% chance of approval. Additional weeks went by and I was called in. The student and advisor told me it was denied. Of course I was extremely disappointed, but I was advised it could fall in the 10%. I then asked the student if he could put some bonding on the two front teeth because my daughter was getting married and I couldnt have a missing tooth. They told me that my jaw had adjusted and now there was no room to put anything on the back. I have had many 3D X-rays and it never mentioned that things were changing in my jaw alignment. They also told me that I would no longer be a patient at their facility, despite the fact they started bone regeneration despite the fact I was scheduled a month from that visit for surgery to finish an implant. I was just escorted to the front receptionist to get my records, as it would be my last visit. I did not pick them up that day because I was devastated by the whole experience. I felt physically sick.I sent an email explaining this whole thing. (Email verified it was receiveno one reached out. I then took to online review sources and their representative left a message to my complaint that they deal with things online. (I put in my post that I had sent an email first). I am going to consult an attorney.
