The Oregon Secretary of State has issued the following Scam Alert stating
"Oregon businesses are reporting receiving a questionable solicitation entitled "2018 - Annual Report Instruction Form." This form, sent by "Workplace Compliance Services" can be easily mistaken for correspondence from the State of Oregon.
Despite its appearance, the Annual Report Instruction Form was not sent by the Secretary of State Corporation Division. The address listed to return a check is in fact a UPS store: 2755 Commercial St. SE, Ste. 101-260, Salem.

Instruction Form” to businesses in Hawaii urging them to complete the report
per requirements specified in Hawaii Revised Statute. As of June 25, 2018, BBB
is not able to verify that Workplace Compliance Services is licensed and
registered to do business in the State of Hawaii. This company’s advertised
address, 4348 Waialae Ave, #700, Honolulu, HI 96816, is a UPS Store mail box
address.
