    On Saturday, July 8, 2023, I received a debt collection letter from Collection Associates, LTD. It states that I owe $5995 for a bill from a durable equipment supplier with a date of June 2020. I have never received a bill from any equipment supplier before or after that time. I called the collection agency and they stated it was from a durable medical company but no name given. The company listed on the letter is not one that Ive ever heard of nor made any contract with. I have sent an email to request the copy of the bill along with the original company. I called the hospital where I had a procedure done in May of 2020. They said I owe nothing. They contacted the durable medical people they use for supplies and that company called me stating they do not show me owing anything to this company and that this company sending out the letter is currently being investigated by the federal government. He said that this company most likely got information on patients from a 3rd party and its a scam. I want to know if this company is a legit company and if so, I want to know the original company so that I can deal with them directly, especially since the original company that was used by the hospital is saying that I owe nothing. If I owe something, I pay it. But in this case, I have nothing that needs paid.

    I had my identity stolen in 2021, I have NEVER done business, had any business or heard of this company until they fraudulently begun reporting to the credit bureaus on or around 06/13/2023. This company has broken FCRA laws and are reporting fraud to *********** as of this complaint being filed today, 06/19/2023. I was never notified by phone, in writing or any other contact regarding this matter. I am working with a federal attorney and judge regarding ongoing ID theft matter at this time. I demand this be REMOVED from any reporting agency at once.

    On September 15th 2020 I was seen at advanced pain management. I issued a check to pay my self pay for the visit for $126.00. I also paid $100 to advance pain management on December 23rd 2020 which was my last payment made to advanced pain management before the company closed. I have only ever had self pay and never received a bill in the mail for advanced pain management because I was self Pay. March 2nd 2023 I received a letter stating that I owed $239.04 including interest on a past due bill for advanced pain management. The bill, they stated in a phone call I made to them, is from September 15th 2020. This debt does not show up on my credit report either. I also understand from the letter that my state tax refund is being held because of this debt. I called their offices to try and settle this and provide proof that the bill has been paid on the date of service. They argued that I had to provide proof of a bill, which I never received because I had already paid it on the date of service.

    Collection associates, *** (acting for APM Receivables, LLC) is saying that I owe money from a service provided from a closed business (Pain Centers of *********, ********************) that closed about 3 years ago.All payments were completed to this company and I never received any bills from them showing I owed additional money. Pain Centers of ********* is apparently out of business and I am unable to contact them.The amount they say is owed is $50.00 It appears that Collection Associates, *** has erroneous information or is simply trying to scam me.
