    1. During 2023, and partially into 2024, employee *** payments were directed into the wrong account. No accountability taken from Payroll Vault. 2. In mid-October 2023, Payroll Vault did a payroll test run with 2024 pay rates in order to come up with the 2024 employer share p/r tax amounts for budget purposes. When the actual payroll for Oct '23 was run, the pay rates were not returned to 2023's numbers, but it was run with the proposed rates for 2024. No accountability taken from Payroll Vault. 3. ****** SUI payroll tax was calculated at the wrong rate ******* 2024. P/R Vault made the corrections after months of pushing them to do so.4. For all of 2023 and the first two quarters of 2024 P/R Vault calculated and paid the ***** tax at the wrong rate. They would not correct the matter and took no accountability. This resulted in penalties that Payroll Vault did not cover. This also meant we had to pay our bookkeeper additional time to fix the error that was created by Payroll Vault. This resulted in an additional cost of $326.50 to Comeback Yoga for her time. 5. FUTA Tax was deducted in error by Payroll Vault *********. Payroll Vault was asked to fix the error in January 2024 and as of May 2, 2025 the return of the erroneously deducted money has yet to happen.
