    I have received an email from Icon Dental stating that I have a payment of $679.32 due to Icon Dental. However, it's contradicting the info I obtained from the dental insurance, Guardian.The overpayment is due to missing claims submission by ******************************* from Icon Dental. If you review the EOBs statements and the email attached, you would see ******** had never submitted claims for ortho visits on 08/31/2018, 10/12/2018, 12/06/2018, and 12/28/2018. Even for my other ortho visits in 2018 and 2019, they were not submitted until October 2020. This was far beyond any insurance company claim submission policy. In the attached email, I highlighted the section where ******** did not submit 2018 and 2019 claims timely. Please keep in mind that ***** visits on 08/31/2018, 10/12/2018, 12/06/2018, and 12/28/2018 had never been submitted. I had to email back and forth with ******** with about 40 emails, to escalate her submitting my claim. Guardian originally was not planning on covering late claims due to Icon Dental's late submission, however, Guardian paid the full $2000 benefit somehow in October 2022. Now after Guardian reviewed the case, they realized they should not have paid the full benefit for the late submitted claims. That's why they requested payback of $679.32 in February 2022 from Icon Dental. However, as I stated multiple times with ******** in the email, it's not my responsibility to guide or escalate her submitting insurance claims. So it's not my responsibility to pay $679.32, as the Icon Dental billing office had not submitting all claims in 2018. Attached includes:1. Most recent communication that Icon Dental threatens to sent the bill to collections;2. Billing Questions Email with ******************************* since Aug 2020 3. All EOBs from Guardian that shows no claims submitted for ***** visits on 08/31/2018, 10/12/2018, 12/06/2018, and 12/28/2018.Appreciate your time and help in assisting this compliant!
