it started on 09/25/23 ive been on medical leave from heart attac my employer reqires certain form to be filled out for me to recieve my benifits . my employer cut my benifits off at 9/25 i havent gotten paid in over 6 weeks they have been contacted by me my employer and nothing i was to return to return to work on 11/05/23 but cant becase they wont send in my medical release form that i sent to my doctor im about to loose every thing becuse my benifts stopped and no i about to loose my job i even paid to have paper woork sent nothing i have overdraft fees and lost hear recovering from heart atrack and dealing with this stress havent been able to get my meds im suposed to take these guys destroyed my life over not replying with the paperwork ive been out since 08/11/23 these guys need to be prosicuted the amount im putting in next box is **** bank fees and paymentsBusiness response

Filed FMLA paperwork with this company as a third party company for my surgeon's office on 10/16. This company reached out to me to pay their fee, which cleared my bank on 10/16. This company claimed multiple times the forms would be completed in no more than 5 business days. I have yet to receive the completed forms, and I can't even get a legitimate updated on the forms besides they will look into it and have someone contact me. I have yet to be contacted by anyone from this company since I paid the bill. Nobody can tell me what they are lacking for the forms to be complete, or when I can expect them to be completed. They do acknowledge with each call that it is past the deadline that they should have been completed. I have yet to be able to get any type of supervisor, customer service rep, or manager on the phone. They instead place me on hold for longer than 30 minutes at times and keep coming back to tell me they are trying to find someone available to talk to me. I am now out money that this company charged me, and they have yet to complete their service for the payment. I am now 4 days away from my surgery and don't have the forms to turn into my work that was due 10/20 for requested time off for my surgery. This business is proving to be taking advantage of vulnerable people by taking their money and not rendering the service. This company is causing a delay in patient care and is completely unacceptable.Initial Complaint

8/14/2023 Failed to fill out all forms needed for FMLA Call to clarify and was told that not all forms were faxed but my doctors office faxed and scanned them into their system in front of me, so I know they were all sent. Being told it will take **** day to get these forms filled out. I've already paid now and waited. I just need to get these forms filled out.Business response

It is never our intention to purposely detain or cause a delay in the delivery of a medical form. Our focus is to treat every medical form with the care and urgency it deserves, while complying fully with Health Insurance Portability and Accountability Act (HIPAA) guidelines for appropriate security and privacy.Forms were received 8/21 and completed 8/21 at no charge to the patient. We contacted the customer to provide additional details and assist with their concerns.Customer response

I filed an FMLA extension and a ADA ************* on 6/27. Paid 6/29 for both forms. BOTH forms were filled out incorrectly and had to be redone. I had ***** for each form. It has been nothing but an absolute mess! I have spoken to three different supervisors who promised they would update me with a resolution- no call, no email from all 3. Yesterday I was told I would get a call from the person who fills out my paper work to cut the middle man out and to get a resolution. Yet again, no call yesterday, no call again today. They keep saying they are looking into the issue. I just need to speak with the person who fills out my paperwork to get the form corrected! Ive already paid. This is time sensitive and my employer has given a deadline and these people have no urgency, no cares.Business response

FLMA paperwork was submitted on 6/22/2023. Sharecare claims to not have received it until 6/26/2023. As of this writing on 7/6/2023 (day 7), paperwork has not been filled out and returned. This is causing problems with me returning to work in the fall. I have called several times to Sharecare, and a few of the nurses have called on my behalf also.Business response

Hello,
After researching, this patients form was received and completed within our TAT. It takes ***** hours for us to add the form to our system after patient submits the form. Patient paid for the form on 6/26/2023 and her form was completed and sent back to the clinic for signature and delivery on 07/06/2023 within our TAT on the 7th business day as stated by patient. We went ahead and issued a refund for this form.

FMLA paperwork was submitted through my health care provider prior to my operation on 6/2/23. Sharecare claims to not have recieved it until 6/7/23 and still have not completed my paperwork as of 6/15/23. This is causing issues with my employer and my paycheck as well.Business response

Share Care is a joke I never authorized Integrated Medical Services to release my medical medical records to them or any personal information to them anytime I call they dont speak English they will not get a supervisor on the phone and they will not even give me my records after signing multiple releases for Social Security for ******** and for myself they have violated HIPAA laws by having my personal records without my permissionBusiness response

They charge me 30 dollars to get paperwork filled out then they only send 4 pages of something else that I didn't even request to fill out instead of the 32 pages I sent with half of it filled out by me and nothing was sent to ********. I NEVER gave permission for my information to be falsely posted to ANY organization. I refuse to register for an account with any organization that has such shady business practices. I am requesting immediate removal of the false information posted as my license requires that I am accurately representing myself to my clients/potential clients. You are exploiting my position to gain registrants by intentionally posting inaccurate information and then blackmailing victims into signing up with health grades. This is wrong and needs to be addressed.Business response

Ortho virginia sent fmla/ short term disability physician statement to shareware on 27 Feb 23. I paid service fee of $30.00 on 8 Mar 23. I called on 14 March and was told to try again on 16 March. I waited until 25 Mar. I sent an email to customer service, was asked to verify my account info, because they can't find my paperwork. I sent account info they requested. I called and got bounced around on the phone and then I got hung up on. I received reply in email that they can't help me. Call the fmla phone number, which I had already called. I just want my paperwork back, so I can submit to my insurance for short term disability. No one at this company cares about the customer. If you look at all the complaints on this company, it's ridiculous. They have people's health care information and hold it hostage until you pay them. After you pay, they still don't send your forms. When does this become a criminal case??? Birth date 03/02/1967
