              Hi , To whom it may concern:I am filing this complaint because the dental provider office of doctor ********************** has charge my delta dental insurance $664.40 on 8/24/2023, claiming that they have installed permanant crown on tooth number 18. Smile and Shine dental have not delivered permanant crown for my tooth number 18. Yet they submitted a claim stating that I have received my permanent crown for tooth number 18 on August 24, 2023. This information is falseI, have not received the crown. I would like the amount of $664.40 returned back to my delta dental insurance so I can go get my crown elsewhere or for them to mail me my crown as I dont feel comfortable and safe being in their office and receiving dental care from them.
