Carebridge Innovations, 1515 N University Dr, Ste 214, Coral Springs, FL 33071-6099 FRAUDULENTLY billed my Medicare account in the amount of $11,477.29 for bogus shipments of 1800 urinary catheters that I did not order, received or needed. The claim was billed to Medicare, retroactively, in 6 installments of 300 catheters each, covering a period from May 21 to October 21,2025. THIS FRAUDULENT CLAIM HAS BEEN REPORTED TO MEDICARE.
