Description
Via a UPS received a request for saliva specimen for a Comprehensive Neurology Test. Form had my mother's name, DOB, address, medicare ID. Letter head says Shore Labs LLC, ordering physician Telesolutions MD PC in Riverside CA. Return address is Lab Services 600 Fairway Dr Deerfield, FL. This is 3rd or 4th request from various companies for this type of "genetic" testing.
