            The service I am disputing is an office visit with a primary care provider which occurred on June 16, 2021. I have been charged and just recently paid (in order to prevent going to collections) a **** for $432. The nature of my dispute is the practice unfairly billed for services that were either not requested/not necessary, or not even received The business committed to provide an annual wellness exam for preventative care. Instead, when I arrived, I was told this would be a new patient visit, and I would have to have a separate visit for my wellness exam. I was told I had to complete this visit before returning for my actual physical exam. I assumed since it was a requirement for the annual wellness visit, it would fall under preventative care, and be covered by my insurance company. Though I had no specific health issue I was seeking care for, the business used health questionnaire forms I completed to identify "diagnoses" of conditions which they used to code for on the billing claims. Furthermore, I was charged for a new patient visit "level of service" equating to ***** minutes of time. I attempted to appeal this with my health insurance, but the insurance provider (Regence) indicated they could not provide more coverage because the nature of the coding indicates I was seeking care for specific conditions. When I called (over 6 different attempts in the last 9 months) to speak with the billing department at the practice to request a re-coding of the encounter, I could only reach an external third party billing service, which had to then communicate with the practice. I never received a response back from the practice, just more bills and finally a threat that the outstanding balance would go to collections.Account #*****

            Their billing department is absolute trash, they are HORRIBLE at being able to answer the phone, I have been dealing with a billing issue for about FIVE MONTHS NOW, even left multiple messages through their portal. I have had to call them repeatedly and when they finally answers I keep having to explain that I have been billed incorrectly and that if they look at their records for the last three years I have had the same service every visit and I made it explicitly clear my insurance only covered 45 minutes not an hour and for a long time I had no issue. But now I'm over 100+ days "late" on a **** I should not be paying for. When I last spoke to someone in billing and explained to them that someone else in billing said they would take care of this and not to worry, she said oh well lemme put a rush on this claim so we can take care of it. HOW WAS IT NOT ALREADY HANDLED?? I JUST received a letter from my insurance about the claim and I am baffled when it is THEM who billed it incorrectly so why does my insurance need to look into this?? I have now received yet another **** from SWFP including late fees. How has it taken 5 months to process that you guys messed up. For this reason I am never returning, how incredibly stressful and unprofessional.
